Materials We Request

Prior to our first session together, we request that you provide us any applicable items from the list below.  By permitting us to obtain such information we are better equipped to provide you with personalized advice and information to help you meet your specific financial objectives.  Access to this information in advance of our meeting allows our team to make the preparations required for this complimentary session.  As a result, we are able to make the most efficient use of our time together during this initial engagement.

The materials we request for our Personal Mission Builder engagement include the following: 

1. Completed Personal Mission Builder Worksheet 
2. Most recent paystub and/or details for all sources of income
3. Recent statements for financial accounts such as bank, brokerage, retirement and 529 plans
3. Most recent federal tax return (1040)
4. Copies of any legal documents including Wills, Living Wills, and Powers of Attorney
5. Details of any existing life insurance policies, including employer group coverage.
